Just mic the old flap and get something equivalent. Instead of rubber get a Viton or Silicone sheet....more expensive but will hold up much better.

Default Alternate part that works

For everybody looking for this part (15209785) I found an alternate part from Dodge that is nearly an exact match, just search for Mopar 68260545AA. The only thing is you need to pry off the outer vent portion of the old part that faces the spare tire carrier with a utility knife and glue it onto the new part. Now I can remove the rag I have stuffed into the hole. I found this image from some russian dude who manually repaired his (pretty good job)

Just wanted to give a shoutout to @superbeer for the tip about the rear door vent. I just finished doing this and can confirm that everything works as described. The structural components of my vent were fine, but the rubber seal which held it against the door was 75% gone which caused it to rattle around when I accelerated mainly. The plastic piece that you pry off the old unit is basically a perfect fit when you glue it to the new vent. I added some photos of the finished product for reference below.

I have a 2008 and went with the Quarter Panel Exhauster - Mopar (68260545AA) for $18 (part + shipping). Ordered from them Thursday evening at 6:00 and the part was in my mailbox this afternoon (Monday).
