Yes the airdoc replaces the intake tract leading from the stock closed box, up to the intake. It also eliminates the resonator which absorbs a good deal of power/mileage at an attempt to keep the truck quiet and "civil".

Amazingly, I do often have intelligent things to say. People just ignore the good and jump on the bad.

Interesting.. doc reffers to it as a resonator also:
The lungs of this intake? A 3.5" diameter steel TIG welded pipe with mandrel bend design. Doc's system makes for a large volume of air flow from the inner fender outside the under-hood heat environment direct to the throttle body. To do this, you replace stock intake hoses, plastic resonator box and air filter box inlet snorkel with the Air Doc.

Well no that was a bit of an exaggeration. Airdoc+k&n plus perhaps synthetic fluids and a tuner will certainly be 23-ish mpg though.
