I thought that thing was set on bearings, any chance they are simply worn out and need to be replaced?

you're going to need to take it off, partially anyway. the problem with the old style carriers is the lack of room inside the cylinder for grease to get in. changing the fitting won't help. you can take off the spare, or leave it on, but you'll need to lift the carrier out of the socket that's bolted to the bumper. it takes some muscle, that's why i had my wife do it, but just lift it straight up, eventually it will come out. clean all the crap out of the socket and off the carrier peg. grease them both up really well, preferably with synthetic (seems to hold up better against the crud that builds up), then put it back together. you'll have plenty of grease spooge out on top, but don't get crazy trying to clean it off. a few shop rags will work, but don't spray any kind of degreaser on it, otherwise you'll end up back where you started.

No spare tire needed...

I bought my 2003 H2 new and it had the single rear seat and spare tire in the back... taking up almost all the space. I took out the little seat and stored it in the garage. Same with the spare tire... I did not want the outside mount spare tire in the way all the time. After 198,000 miles I have never needed to change a tire on the road. I did have to do a two plugs from nail punctures. And I carry a good commpressor. Road side assistance is everywhere if a tire has a blowout. I did a few hours of offroad driving the first year, but never again. The spare tire and seat weigh more than my rooftop tent.

No spare, no seat, bed, toiler, speaker, backpack
